I came to functional medicine partly because of this pattern – and partly because I lived it myself. Years ago I developed SIBO, persistent bloating, and a tiredness that no amount of sleep seemed to shift. I was doing everything my conventional training suggested. The labs were borderline normal. The energy was not.

That experience sent me deep into the research on energy, inflammation, and the nervous system. And what I found there changed the way I understood not just gut health – but tiredness itself.

What they describe follows a pattern I now recognise immediately.

Here is something that took me years in practice to fully understand – and that I rarely see explained outside specialist circles.

Energy isn’t just about what you put in. It’s about whether your cells can actually use it.

When the body is under chronic stress – from work pressure, caregiving, family demands, or simply years of not fully recovering – the immune system produces inflammatory messengers called pro-inflammatory cytokines (small proteins that signal threat and activate the body’s defence systems).

Think of cytokines as the body’s alarm system. They are designed to help you survive short-term threats. The problem begins when the alarm never switches off.

In short bursts, this is useful. In chronic activation, it becomes a problem.

These cytokines don’t just cause inflammation. They actively block the mitochondria – the energy-producing structures inside your cells – from functioning properly.

You can think of mitochondria as tiny power stations inside almost every cell in your body. When they work well, you have energy. When something blocks them, no amount of sleep or nutrition changes that.

This is why exhausted people can sleep 9 hours and still feel depleted. This is why supplements can raise levels on a blood test but not raise energy in a real person. The problem isn’t input – it’s the system processing it.

The body has its own mechanism for regulating these cytokines and restoring cellular energy production. And chronic stress – in any form – appears to impair this regulatory system over time.

Which left me with one question I couldn’t stop thinking about:

Is there a direct way to reach that regulatory system?
